Medical Information

About This Medicine

Anusol HC is a medicated treatment for haemorrhoids (piles) and other anorectal conditions that combines anti-inflammatory, astringent, and protective properties. It is available in both suppository and cream formulations and is used to relieve the pain, itching, swelling, and discomfort associated with internal and external haemorrhoids. Anusol HC contains hydrocortisone, a mild corticosteroid, alongside other active ingredients that help soothe and protect the sensitive anal and rectal tissues.

What Anusol HC Treats

Haemorrhoids are swollen, inflamed blood vessels in and around the rectum and anus. They can cause significant discomfort, itching, bleeding, and a feeling of fullness or prolapse. Anusol HC is indicated for the symptomatic relief of uncomplicated haemorrhoids, pruritus ani (anal itching), and mild anorectal inflammation. It is not a treatment for the underlying cause of haemorrhoids but provides effective short-term relief while lifestyle modifications and, where necessary, procedural treatments address the root problem.

How Anusol HC Works

The hydrocortisone component in Anusol HC exerts anti-inflammatory, antipruritic, and vasoconstrictive effects on the anorectal mucosa, reducing swelling, itching, and irritation. Alongside hydrocortisone, the formulation typically contains bismuth oxide and bismuth subgallate, which act as astringents to reduce secretions and soothe irritated tissue. Zinc oxide provides additional protection and barrier function over the sensitive skin. Peru balsam has mild antiseptic properties. Together, these ingredients create a product that addresses multiple aspects of haemorrhoidal discomfort in a single application.

Anusol HC should only be used for short periods, typically no longer than seven days, under the guidance of a pharmacist or doctor. Prolonged use of topical corticosteroids in the perianal area can cause skin thinning and other local adverse effects. If symptoms persist beyond a week, a medical review is essential to rule out other anorectal conditions.

Usage & Dosage

How to Use Anusol HC

Suppositories: Wash your hands before use and empty your bowels if possible. Remove the foil and moisten the suppository with a little water. Lie on your side with one knee drawn up and gently insert the suppository pointed end first into the rectum. Use one suppository in the morning, one at night, and one after each bowel movement, up to a maximum of four per day.

Cream: Apply to the affected external area, or into the rectum using the provided applicator, morning and night and after each bowel movement. Clean the area gently before each application. Do not use Anusol HC for more than seven consecutive days without medical advice. If symptoms do not improve, or if bleeding is present, see your doctor.

The recommended dosage of Anusol HC suppositories for adults is one suppository inserted into the rectum up to four times daily: in the morning, at night, and after each bowel movement. The maximum recommended treatment duration is seven days.

For the cream formulation, a small amount should be applied to the affected area two to three times daily and after each bowel movement. The cream may be used internally using the applicator provided.

Anusol HC is not recommended for use in children under 12 years of age without medical supervision. Elderly patients may use the standard adult dose but should be monitored for signs of skin thinning with prolonged use.

Patients with impaired liver or kidney function do not typically require dose adjustment for short-term topical use, as systemic absorption of topical hydrocortisone at these concentrations is minimal. However, if a patient is already receiving systemic corticosteroid therapy, the addition of topical hydrocortisone should be discussed with a doctor to avoid excess steroid exposure. Treatment should not exceed seven days; if a longer course is considered necessary, medical review is required.

Side Effects

Anusol HC is generally well tolerated when used as directed for short periods. Side effects are more likely with prolonged or excessive use.

Common Side Effects

The following local effects can occur, particularly with extended use:

  • Burning or stinging sensation at the site of application
  • Local irritation or increased itching
  • Skin redness or sensitivity reactions
  • Dryness of the perianal skin
  • Folliculitis (inflammation of hair follicles) with prolonged cream use
  • Mild allergic contact dermatitis

Serious Side Effects

The following adverse effects are associated primarily with prolonged or excessive use of topical corticosteroids and require medical attention:

  • Skin thinning (atrophy): perianal skin can become fragile and prone to tearing with extended use
  • Striae (stretch marks) in the perianal region
  • Adrenal suppression: particularly relevant if large areas are treated under occlusion or in children
  • Secondary skin infections: corticosteroids can mask or exacerbate existing bacterial, fungal, or viral infections
  • Allergic hypersensitivity reaction to any component of the formulation

Discontinue use and consult your GP if you notice increased irritation, worsening symptoms, visible skin changes, or signs of infection such as increased redness, pus, or fever.

Warnings & Precautions

Skin Safety and Duration of Use

Do not use Anusol HC for more than seven days without medical advice. Prolonged application of topical corticosteroids to the perianal skin can cause irreversible thinning of the skin, making it fragile and susceptible to injury. This is particularly important in this sensitive anatomical area. Children's skin is more permeable and susceptible to corticosteroid absorption; Anusol HC should not be used in children under 12 without a doctor's guidance. Avoid applying the cream under occlusive dressings, as this increases absorption significantly.

Pregnancy, Breastfeeding, and Infections

Anusol HC should be used with caution during pregnancy. Topical corticosteroids have been shown to have fetotoxic effects in animal studies, and while the extent of systemic absorption from short-term perianal use is low, it should only be used during pregnancy when clearly necessary and for the shortest possible time. Consult your midwife or GP before use. Breastfeeding mothers should also seek advice before using Anusol HC. This product should not be used in the presence of infected haemorrhoids or perianal infections caused by bacteria, fungi, or viruses, as corticosteroids can suppress the immune response and allow infections to worsen. If perianal bleeding is a symptom, medical assessment is required before initiating treatment to exclude other causes such as colorectal polyps or carcinoma.

Contraindications

Anusol HC is contraindicated in the following circumstances:

  • Known hypersensitivity to hydrocortisone, bismuth salts, zinc oxide, Peru balsam, or any other component of the formulation
  • Perianal infections caused by bacteria, fungi, viruses, or tuberculosis
  • Rosacea, perioral dermatitis, or acne in the application area
  • Children under 12 years of age without medical supervision
  • Use as a primary treatment for bleeding haemorrhoids without medical evaluation
  • Viral skin infections, including herpes simplex, in the application area
  • Prolonged use beyond seven days without medical review
  • Patients with known allergy to Peru balsam or related compounds (cross-reactivity with some fragrances and topical anaesthetics possible)

Frequently Asked Questions

How long does it take for Anusol HC to work?
Many patients notice some relief from itching and discomfort within the first 24-48 hours of starting Anusol HC. Inflammation and swelling typically begin to reduce over the first few days of consistent use. If there is no improvement after seven days, you should stop using the product and consult your GP.
Can I use Anusol HC during pregnancy?
Anusol HC contains hydrocortisone, a corticosteroid, and should only be used during pregnancy if clearly necessary and for the shortest possible time. The potential for systemic absorption from short-term perianal application is low, but you should speak to your GP or midwife before using it to ensure it is appropriate for your individual situation.
Is Anusol HC available without a prescription?
Anusol HC is available over the counter at pharmacies in the UK for adults. However, a pharmacist will ask you questions to make sure it is suitable for you before supplying it. If you have any uncertainty about your symptoms, or if rectal bleeding is present, you should see your GP before starting treatment.
Can I use Anusol HC every day for a long time?
No, Anusol HC should not be used continuously for more than seven days without medical advice. Prolonged use of topical hydrocortisone can cause thinning of the perianal skin, which can make symptoms worse in the long term. If your haemorrhoids require ongoing treatment, speak to your GP about alternative management options.
What should I do if my symptoms include rectal bleeding?
Rectal bleeding should always be assessed by a GP before you start self-treating with Anusol HC. While bright red blood on the toilet paper is often caused by haemorrhoids, bleeding can also indicate other conditions such as anal fissures, polyps, or, more rarely, colorectal cancer. Do not assume bleeding is simply due to haemorrhoids without a professional assessment.
