About This Medicine
Atacand Plus combines candesartan, a potent angiotensin II receptor blocker, with hydrochlorothiazide, a thiazide diuretic, in a single tablet for the treatment of hypertension. This combination targets two different mechanisms of blood pressure regulation, producing a larger reduction than either component alone.
Why combine two antihypertensives?
Candesartan blocks the renin-angiotensin system, causing vasodilation and reduced aldosterone secretion. Hydrochlorothiazide reduces blood volume by increasing sodium and water excretion from the kidneys. The two mechanisms are complementary, and the combination also mitigates each drug's compensatory responses: the diuretic-induced activation of the renin-angiotensin system is counteracted by candesartan, while candesartan's tendency to retain potassium offsets the diuretic's potassium-wasting effect.
When is Atacand Plus prescribed?
Atacand Plus is used when blood pressure has not been adequately controlled by candesartan or hydrochlorothiazide alone, or as initial therapy in patients whose blood pressure is substantially above target and likely to require more than one agent.
Usage & Dosage
How to Take Atacand Plus
Take one Atacand Plus tablet once daily at the same time each day, with or without food. Check your blood pressure regularly after starting to confirm an adequate response.
Hydration
The diuretic component of Atacand Plus increases urine output, particularly in the first few days. Maintain adequate fluid intake, especially in hot weather or during vigorous exercise. Let your doctor know if you feel dizzy or excessively thirsty, as these can be signs of dehydration.
Available as candesartan 16 mg / hydrochlorothiazide 12.5 mg. One tablet once daily. The prescriber may have previously titrated each component separately before switching to the combination tablet.
Side Effects
Common side effects
- Dizziness
- Headache
- Increased urination
- Fatigue
- Hypokalaemia (less common than with diuretic alone)
Metabolic effects
Hydrochlorothiazide can mildly raise blood glucose, uric acid, and cholesterol levels. These effects are generally small at the 12.5 mg dose used in this combination.
Warnings & Precautions
Monitor renal function, electrolytes, and blood pressure regularly. Not suitable in pregnancy. The diuretic component can cause photosensitivity in some patients.
Dehydration risk
Illness causing vomiting or diarrhoea can lead to dehydration and a sharp drop in blood pressure when combined with this medication. Seek medical advice if unable to maintain fluid intake during illness.
Contraindications
Contraindicated in pregnancy, anuria, severe renal impairment, severe hepatic impairment, refractory hypokalaemia or hypercalcaemia, and known hypersensitivity to candesartan, hydrochlorothiazide, or sulphonamide-derived drugs.
