Flomaxtra XL is a once-daily modified-release tablet containing tamsulosin hydrochloride 400 micrograms, prescribed for the treatment of the functional symptoms of benign prostatic hyperplasia (BPH) in adult men. BPH is a non-cancerous enlargement of the prostate gland that becomes increasingly common with age, affecting over half of men aged 60 and the vast majority of men in their eighties. As the prostate enlarges, it compresses the urethra and increases smooth muscle tone in the bladder neck, leading to lower urinary tract symptoms (LUTS) including difficulty initiating urination, a weak or intermittent urine stream, a feeling of incomplete bladder emptying, increased frequency, and urgency.
How Flomaxtra XL Relieves Urinary Symptoms
Tamsulosin is a selective alpha-1a adrenoceptor antagonist. Alpha-1a receptors are predominantly located in the smooth muscle of the prostate stroma, bladder neck, and proximal urethra. By blocking these receptors, tamsulosin reduces the resting tone of the smooth muscle in these structures, thereby widening the urethral lumen and reducing bladder outflow resistance. This results in improved urinary flow rate and a reduction in obstructive and irritative urinary symptoms. The clinical effects of tamsulosin are generally apparent within the first two weeks of treatment, which is significantly faster than alpha-reductase inhibitors such as finasteride, making it a preferred option when prompt symptom relief is the primary goal.
Modified-release Formulation
The "XL" designation indicates that Flomaxtra is formulated as a modified-release tablet, which ensures a gradual and sustained release of tamsulosin over the course of the day. This modified-release profile not only maintains consistent therapeutic plasma concentrations but also reduces the magnitude of peak plasma levels compared with immediate-release formulations, which is associated with a lower risk of first-dose cardiovascular effects such as postural hypotension. Flomaxtra XL should be taken once daily after breakfast and swallowed whole; the tablet must not be crushed, chewed, or opened.
Usage & Dosage
How to Take Flomaxtra XL
Flomaxtra XL (tamsulosin 400 mcg) should be taken once daily, after breakfast or after your first meal of the day. Taking it with food reduces peak blood levels and lowers the risk of dizziness or a drop in blood pressure. Swallow the tablet whole with a glass of water. Do not crush, break, or chew it — this destroys the modified-release mechanism and causes the full dose to be absorbed at once.
The dose is fixed at one tablet daily — do not take more than one tablet in 24 hours. Treatment is for long-term symptom management of benign prostatic hyperplasia (BPH). Improvement in urinary flow and a reduction in urinary symptoms are typically noticeable within one to two weeks.
First-Dose Effect
A significant drop in blood pressure on standing (postural hypotension) can occur with the first dose, causing dizziness or faintness. Taking the tablet with food and sitting or lying down for a few hours after the first dose minimises this risk.
The dose of tamsulosin in Flomaxtra XL is fixed at one tablet (400 micrograms) taken once daily after breakfast. There is no titration required with this modified-release formulation, in contrast to some other alpha-1 blockers that require dose escalation. The fixed 400 microgram dose has been shown in clinical trials to provide optimal efficacy and tolerability for the treatment of LUTS due to BPH.
No dose adjustment is required for elderly patients based on age alone. Caution is advised in patients with severe renal impairment (creatinine clearance below 10 mL/min), as there are limited data in this population. Flomaxtra XL is not recommended in patients with severe hepatic impairment, as tamsulosin is extensively hepatically metabolised. It should not be used in patients with a history of orthostatic hypotension. Flomaxtra XL is for adult men only.

Common Side Effects
Tamsulosin is generally well tolerated. Common side effects include:
- Dizziness or light-headedness, particularly on standing (postural hypotension)
- Retrograde ejaculation — where semen passes backwards into the bladder during orgasm rather than forward. This is harmless but should be discussed with your doctor if it concerns you
- Headache
- Weakness or fatigue
Before Eye Surgery
Tell your ophthalmologist or eye surgeon that you are taking tamsulosin before any eye surgery, particularly cataract surgery. Alpha-blockers like tamsulosin can cause a condition called intraoperative floppy iris syndrome (IFIS), which surgeons need to know about in advance to manage safely. Do not stop tamsulosin before surgery unless specifically instructed by your prescriber — suddenly stopping can cause urinary retention.
Warnings & Precautions
Intraoperative Floppy Iris Syndrome - Critical Information
Patients taking Flomaxtra XL who are planning to undergo cataract surgery or any other eye surgery must inform their ophthalmologist that they are taking tamsulosin before the procedure. Tamsulosin is associated with intraoperative floppy iris syndrome (IFIS), a complication in which the iris has reduced tone during surgery, making it prone to billowing, prolapse through surgical incisions, and progressive miosis. IFIS can significantly increase the complexity of cataract surgery and the risk of intraoperative and postoperative complications.
Importantly, IFIS can occur even in patients who have stopped taking tamsulosin weeks or months before surgery, as the drug appears to cause a persistent change in iris smooth muscle. If cataract surgery is planned, ideally it should be performed before starting tamsulosin treatment. If you are already taking tamsulosin and have not yet had surgery, do not stop the drug before surgery without first discussing with both your urologist and your ophthalmologist.
Orthostatic Hypotension and Drug Interactions
Flomaxtra XL should be used with caution in patients also taking phosphodiesterase-5 (PDE-5) inhibitors such as sildenafil, tadalafil, or vardenafil, as the combination can cause significant additive hypotension. Other antihypertensive agents and strong CYP3A4 inhibitors such as ketoconazole or ritonavir can also increase tamsulosin plasma levels and enhance the risk of hypotension.
Contraindications
Flomaxtra XL is contraindicated in the following circumstances:
- Known hypersensitivity to tamsulosin or any tablet excipient, including soya (the tablet contains soya lecithin)
- History of orthostatic hypotension
- Severe hepatic impairment
- Women and children (not indicated for these groups)
- Concurrent use with strong CYP3A4 inhibitors such as ketoconazole in patients who are poor CYP2D6 metabolisers
- Use as a treatment for prostate cancer or other malignant prostatic disease (Flomaxtra XL treats functional symptoms of BPH only)
- Patients with known allergy to peanuts or soya (contains soya lecithin)
