About This Medicine
Tadalafil is a phosphodiesterase type 5 (PDE5) inhibitor prescribed for the treatment of erectile dysfunction and the signs and symptoms of benign prostatic hyperplasia.
It is also licensed, under the brand name Adcirca, for the treatment of pulmonary arterial hypertension.
Tadalafil is the active ingredient in Cialis and is now widely available as a generic medication.
Tadalafil works by inhibiting PDE5, an enzyme that degrades cyclic guanosine monophosphate (cGMP) in the smooth muscle of the penile vasculature.
During sexual stimulation, nitric oxide is released locally, activating cGMP production.
By preventing its breakdown, tadalafil enhances the vasodilatory effect, increasing blood flow to the corpus cavernosum and facilitating an erection.
Tadalafil is distinguished from other PDE5 inhibitors by its prolonged duration of action, lasting up to 36 hours.
This extended window has led to it being referred to informally as the 'weekend pill'.
It is available in both on-demand and daily dosing regimens, offering flexibility depending on individual preference and clinical need.
Usage & Dosage
For on-demand use: take one tablet at least 30 minutes before anticipated sexual activity. The effect may last up to 36 hours, but sexual stimulation is still required for an erection to occur.
Do not take more than one dose per day. For daily use: take one 2.5 mg or 5 mg tablet at the same time each day, regardless of sexual activity. Tadalafil may be taken with or without food.
Avoid excessive alcohol consumption as it may reduce the ability to achieve an erection and may increase the likelihood of side effects.
For erectile dysfunction on demand: the recommended starting dose is 10 mg, taken at least 30 minutes before sexual activity.
Based on efficacy and tolerability, the dose may be adjusted to 20 mg or reduced to 5 mg. Maximum one dose per day. For daily use: 2.5 mg to 5 mg once daily.
For benign prostatic hyperplasia: 5 mg once daily. In patients with moderate renal impairment, start with 5 mg on demand; in severe renal impairment, the maximum is 5 mg every 48 hours.
Side Effects
Very common (more than 1 in 10): headache. Common (1 in 10 to 1 in 100): back pain, myalgia, nasal congestion, flushing, dyspepsia, gastro-oesophageal reflux, pain in extremity.
Uncommon (1 in 100 to 1 in 1,000): dizziness, blurred vision, tinnitus, tachycardia, hypotension, epistaxis, abdominal pain, rash, hyperhidrosis.
Rare (1 in 1,000 to 1 in 10,000): priapism (prolonged painful erection lasting more than 4 hours), sudden hearing loss, non-arteritic anterior ischaemic optic neuropathy (NAION).
Seek urgent medical attention for priapism or sudden visual or hearing changes.
Warnings & Precautions
Tadalafil must not be taken concurrently with nitrates or nitric oxide donors (such as amyl nitrite) in any form, as the combination may cause a severe and potentially fatal drop in blood pressure.
Use with caution in patients taking alpha-blockers for hypertension or benign prostatic hyperplasia.
Patients with significant cardiovascular disease should have a cardiac assessment before commencing PDE5 inhibitor therapy.
Discuss any anatomical penile deformity or conditions predisposing to priapism with your prescriber.
Contraindications
Tadalafil is contraindicated in patients taking nitrates or nitric oxide donors, those with severe cardiovascular disease (such as unstable angina or recent stroke or myocardial infarction within the past 90 days), patients with uncontrolled hypotension or hypertension, those who have lost vision in one eye due to NAION, and patients with severe hepatic impairment.
Hypersensitivity to tadalafil or any excipient is also a contraindication.
