About This Medicine
Cialis Once-a-Day contains tadalafil at a low daily dose, prescribed for the treatment of erectile dysfunction (ED) in men who anticipate regular sexual activity (at least twice per week).
By taking a small dose every day, a steady level of tadalafil is maintained in the bloodstream, allowing for spontaneous sexual activity without the need to plan around tablet timing.
Tadalafil is a phosphodiesterase type 5 (PDE5) inhibitor.
During sexual stimulation, nitric oxide is released in the erectile tissue, activating guanylate cyclase and increasing cyclic guanosine monophosphate (cGMP), which relaxes smooth muscle and allows blood flow into the corpora cavernosa.
Tadalafil inhibits PDE5, the enzyme that breaks down cGMP, thereby sustaining the erectile response. Sexual stimulation is still required for the drug to work.
The once-daily regimen offers the advantage of removing the need to anticipate sexual activity.
Clinical trials have demonstrated sustained improvement in erectile function, sexual satisfaction, and overall quality of life with continuous daily dosing.
Usage & Dosage
Take one tablet at the same time each day, with or without food. Swallow it whole with water. Do not take more than one tablet per day.
It may take up to five days of daily dosing for the steady-state level to be reached, so some men may not notice full benefit immediately. Sexual stimulation is necessary for an erection to occur.
Do not combine Cialis Once-a-Day with on-demand PDE5 inhibitor doses or with other erectile dysfunction medications.
If the daily dose proves insufficient, discuss alternatives with your prescriber rather than exceeding the recommended amount.
The usual starting dose is 2.5 mg once daily. If this is insufficient and well tolerated, the dose may be increased to 5 mg once daily. The 5 mg dose is the maximum for daily use.
No dose adjustment is needed for mild renal impairment or mild hepatic impairment. In moderate renal impairment, start at 2.5 mg.
Tadalafil daily dosing is not recommended in severe renal impairment requiring dialysis. In moderate hepatic impairment, use with caution at 2.5 mg.
Side Effects
Common side effects (1 in 10 to 1 in 100 patients) include headache, back pain, myalgia (muscle aches), nasal congestion, flushing, and dyspepsia.
These effects are generally mild and tend to diminish with continued daily use as the body adjusts.
Uncommon effects include dizziness, blurred vision, tinnitus, palpitations, gastro-oesophageal reflux, and rash.
Rare but serious adverse effects include priapism (a prolonged painful erection lasting more than four hours, requiring emergency treatment), sudden vision loss in one eye (non-arteritic anterior ischaemic optic neuropathy, or NAION), and sudden hearing loss.
These events are very rare. If you develop a persistent erection, sudden visual or hearing changes, or chest pain during sexual activity, seek immediate medical attention.
Warnings & Precautions
Cialis Once-a-Day must not be taken with nitrate medications (such as glyceryl trinitrate spray, isosorbide mononitrate, or recreational nitrate drugs known as poppers) due to the risk of a severe and potentially fatal drop in blood pressure.
Avoid concurrent use with alpha-blockers until stabilised, as the combination may cause symptomatic hypotension.
Caution is needed in patients with cardiovascular conditions in whom sexual activity is inadvisable. Report any sudden visual or hearing disturbances immediately.
Contraindications
Tadalafil is contraindicated in patients taking nitrates in any form, those with severe cardiovascular disease in whom sexual activity is inadvisable, recent stroke or myocardial infarction (within the last 90 days), unstable angina, severe hepatic impairment, hypotension (blood pressure below 90/50 mmHg), uncontrolled hypertension, and known hypersensitivity to tadalafil or any excipient.
It is not for use by women or persons under 18.
