About This Medicine

Terra-Cortril is a combination topical preparation containing hydrocortisone, a mild corticosteroid, and oxytetracycline, a broad-spectrum tetracycline antibiotic.

It is prescribed for the treatment of inflammatory skin conditions complicated by bacterial infection, or where bacterial infection is considered likely.

Common indications include infected eczema, infected insect bites, and other inflammatory dermatoses with secondary bacterial involvement.

Hydrocortisone reduces the redness, itching, and swelling associated with the inflammatory component of the condition, while oxytetracycline provides antibacterial cover against a wide range of Gram-positive and Gram-negative organisms that commonly infect damaged skin.

This dual action allows both the inflammation and the infection to be addressed simultaneously with a single application.

Terra-Cortril is available as a cream and also as an ear and eye preparation (Terra-Cortril Nystatin, which additionally contains nystatin).

The topical skin cream is intended for short-term use, as prolonged application of antibiotics may promote bacterial resistance.

Usage & Dosage

Apply a thin layer of Terra-Cortril cream to the affected area two to four times daily. Gently rub it into the skin. Wash your hands after application.

Continue treatment for the duration recommended by your prescriber, typically five to seven days.

Do not use for longer than advised, as prolonged use of topical antibiotics increases the risk of resistance.

If there is no improvement after seven days, return to your prescriber for reassessment.

The underlying diagnosis may need reconsideration, or the infecting bacteria may be resistant to oxytetracycline.

Apply thinly to affected areas two to four times daily. Treatment courses should normally not exceed seven days. If longer treatment is required, your prescriber should reassess.

Do not apply to large surface areas or under occlusive dressings unless directed, as increased absorption of both hydrocortisone and oxytetracycline may result.

In children, use the minimum effective amount and duration.

Side Effects

Common local effects include mild stinging or irritation on application. Oxytetracycline may occasionally cause allergic contact dermatitis or photosensitivity at the application site.

Prolonged use of hydrocortisone, even at mild potency, may cause skin thinning, particularly in flexural areas or under occlusion. Bacterial resistance to oxytetracycline may develop with overuse.

Systemic side effects are very unlikely with short-course topical application to limited areas.

Warnings & Precautions

Do not apply Terra-Cortril to the face for more than five days, as even mild steroids can cause skin thinning and steroid-induced changes on facial skin. Avoid contact with the eyes.

Oxytetracycline may cause photosensitivity; advise patients to protect treated areas from strong sunlight or UV exposure. Do not use on viral skin infections (e.g.

herpes simplex, chickenpox) or fungal infections, as the steroid component may worsen these conditions.

If the infection worsens or the treated area shows signs of bacterial resistance, stop the cream and seek medical advice.

Contraindications

Terra-Cortril is contraindicated in patients with untreated viral, fungal, or tuberculoid skin infections, acne vulgaris, rosacea, perioral dermatitis, and hypersensitivity to hydrocortisone, oxytetracycline, other tetracyclines, or any excipient.

It is not suitable for use in children under two years without specialist supervision.

Frequently Asked Questions

What is Terra-Cortril used for?
It is used for inflammatory skin conditions with a secondary bacterial infection, such as infected eczema or infected insect bites. The steroid reduces inflammation while the antibiotic tackles the bacterial component.
How long should I use Terra-Cortril?
Treatment courses are usually five to seven days. Do not use for longer without medical advice, as prolonged antibiotic use may promote resistant bacteria. If symptoms persist, return to your prescriber.
Can I use Terra-Cortril on my face?
It can be used on the face for short periods, up to five days, if prescribed. Facial skin is more sensitive to steroids, so prolonged use should be avoided. Your doctor will advise on appropriate alternatives.
Does oxytetracycline cause sun sensitivity?
Tetracycline antibiotics can cause photosensitivity. While the risk with topical application is lower than with oral tetracyclines, it is sensible to protect treated skin from strong sunlight during the course.
Can Terra-Cortril be used for fungal infections?
No. The corticosteroid in Terra-Cortril may worsen fungal infections by suppressing the local immune response. If a fungal infection is present, a separate antifungal treatment is needed.
